This position is designed to facilitate an effective process of the Mercy Care Management Model supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient throughput; collaborating with facilities to include leadership coordination as evidenced by metrics, optimizing performance and adoption of best practice.Qualifications: -EducationAssociates Degree in Nursing, requiredBachelors Degree of Science in Nursing, preferred -LicenseRN (Registered Nurse) in the state of practice, required -Experience2 years of clinical experience in case management or related field, required -CertificationsAmerican Heart Association Basic Life Support (BLS) required -OtherDemonstrated leadership skills and excellent clinical competence, including proficiency in nursing improvement processes. Demonstrated excellent interpersonal team-building, collaboration, negotiation, problem-solving, as well as spoken and written communication skills.
