Overview: The Physical Therapist is responsible for delivering
direct rehabilitation services to patients in a post-acute care
environment, adhering to physician referrals and relevant
regulations. This role involves creating and executing therapy
plans, overseeing support staff and students, and ensuring
adherence to safety, legal, and organizational policies. Candidates
must possess licensure, clinical experience, excellent
communication abilities, and actively engage in collaborative
patient care and community health initiatives. Location: Cheyenne,

professional growth Position Summary The Physical Therapist
delivers direct rehabilitation care in accordance with physician
referrals, complying with all relevant laws, regulations, and
organizational standards. Education, Experience, and Licensure
Requirements Graduation from an accredited physical therapy program
(BSPT, MSPT, or DPT; those graduating in 2003 or later must hold an
MSPT or DPT) Current registration/licensure in the applicable
state, with an obligation to maintain an active license in good
standing throughout employment Preferred one (1) year of experience
in a post-acute care or similar setting CPR certification required
upon hire or to be obtained during orientation; must remain current
throughout employment Specific Job Requirements Engage in community
health initiatives and projects as appropriate Proficient in
Microsoft Word, Excel, and email communication Collaborate with
patients, families, and support departments to effectively address
patient needs Exhibit proper body mechanics consistently Make
independent decisions when necessary Knowledgeable about practices,
procedures, and regulations governing operations in a post-acute
care facility Implement and interpret departmental programs, goals,
objectives, policies, and procedures Demonstrate proficiency in all
competency areas, including patient rights, safety, and sanitation
Maintain professional relationships with colleagues, vendors, and
others Ensure confidentiality of all proprietary and sensitive
information Adhere to organizational policies, including those
related to harassment and compliance Exhibit integrity and
professionalism by following the organization's Code of Conduct and
completing mandatory compliance training Essential Functions
Implement physical therapy treatment plans for patients Set,
evaluate, and adjust realistic, measurable, timely, and functional
goals Supervise and assess care provided by PTAs, Rehab Aides, and
PT and PTA students Document care accurately and in a timely manner
Utilize therapy software effectively and accurately Provide
excellent customer service and maintain a positive attitude towards
patients Assist in the safe evacuation of patients Demonstrate
reliable and consistent attendance Employ reasoning skills and
sound judgment Communicate and collaborate effectively within an
interdisciplinary team Engage in physical activities such as
sitting, standing, bending, lifting, pushing, pulling, stooping,
walking, reaching, and moving intermittently during work hours
